If you have a DVA Gold Card, White Card, or TPI Gold Card, you may be able to access Exercise Physiology (EP) services at no cost to you. My aim is to keep the process simple, supportive, and stress‑free so you can focus on your health — not the paperwork.
1. Check Your Eligibility
White Card:

Covers treatment only for the accepted conditions listed on your card.

Your GP referral must link your EP treatment to one of these conditions.
Gold Card:
-
Covers clinically necessary treatment for any health condition.
TPI Gold Card:
-
Covered for all conditions, just like a regular Gold Card.

Exempt from the Treatment Cycle for EP.

Requires one referral per year, not every 12 sessions.
2. Visit Your GP for a Referral
To begin EP services, you’ll need a referral from your usual GP.
Your GP can use:

The D904 referral form (see attached to this page), or
-
A referral letter on clinic letterhead addressed directly to the Exercise Physiologist.
Your referral must include:

Your full name and DVA file number
-
Your card type (Gold, White, or TPI Gold)

The service requested: Exercise Physiology

The condition(s) requiring treatment
-
Essential for White Card holders — only accepted conditions can be treated

Relevant clinical details or comorbidities
-
The name of the specific Exercise Physiologist (not a generic referral)
Tip: Ask your GP to write the condition being treated, not just “Exercise Physiology.”
3. Understanding the Treatment Cycle
Most DVA clients receive EP under the Allied Health Treatment Cycle, which includes:

Cycles of 12 sessions that can be used intensively before a new referral is required, or over a maximum period of 12 months before a new referral is required.

Repeated cycles can be accessed as deemed clinically relevant and necessary between you and your treating GP. At the end of each cycle, your GP will review your progress and can issue a new referral if further treatment is needed. I will provide a report to your GP to help inform this decision.
Exception to the 12-session cycle: TPI Gold Card Holders

Only require one annual referral that covers treatment for any condition.

Your initial session will focus on:

Identifying your goals

Creating a personalized plan to help you move well, build strength, and maintain independence.
