​If you’re living with a long‑term health condition, your GP may be able to refer you to Exercise Physiology under Medicare. This gives you up to five subsidized sessions each year, making it an affordable way to start moving toward better health.

My goal is to help you feel stronger, clearer, and more confident in your body — at a pace that feels right for you.

What Is a Chronic Disease Management (CDM) Plan?

A CDM plan is a care plan your GP creates to help you manage an ongoing health condition. It usually includes:

  • GP Management Plan (GPMP)

  • Team Care Arrangements (TCA) if you need support from more than one health provider

 

These plans help coordinate your care and make sure you’re getting the right support at the right time.

Who Is Eligible?

A chronic condition is anything lasting six months or longer, such as:

  • arthritis

  • diabetes

  • cardiovascular conditions

  • osteoporosis

  • chronic pain

  • cancer

  • neurological conditions

  • long‑term fatigue

  • many other ongoing health concerns

Your GP will decide whether you’re eligible based on your health history and needs.

What You Receive

If your GP prepares a GPMP + TCA, you may access Medicare‑subsidised Exercise Physiology sessions.

2. Get your referral
Your GP will provide a referral specifically for Exercise Physiology.

3. Send your referral
Upload it through the link on my website, or bring it to your first session.

4. Book your appointment
Choose clinic, gym, or home visit — whatever feels right for you.

5. Attend your initial assessment
We’ll explore your goals, your current function, and what matters most to you.
